Posteriorly, the fifth lumbar vertebra (L5) presents a broad laminae meeting in the midline at a short, thick spinous process, with stout pedicles extending anterolaterally toward the vertebral body. Laterally, the transverse processes project from the junction of pedicle and lamina, and the superior articular processes sit posteromedial to them, their facets oriented to receive L4 above. Inferior articular processes descend and angle slightly laterally to articulate with S1, framing the inferior part of the vertebral arch and contributing to the posterior boundary of the vertebral canal. Everything about L5 reads load bearing. This posterior perspective matters because L5 is the keystone at the lumbosacral junction, where facet orientation, pars interarticularis integrity, and the L5 to S1 relationship determine stability under flexion and shear. A defect through the pars interarticularis (spondylolysis) most often involves L5, and when bilateral can permit anterior translation of L5 on S1 (isthmic spondylolisthesis), a common cause of back pain in young athletes and radiculopathy in adults.
