Description
Viewed from an inferior aspect, the right coronary artery arises from the right aortic sinus and tracks within the right atrioventricular (coronary) sulcus between the right atrium superiorly and the right ventricle inferiorly, then curves posteriorly toward the crux of the heart. Along its course, small atrial and ventricular branches are suggested on the epicardial surface, with the right marginal artery typically running toward the acute margin and the posterior interventricular artery often descending in the posterior interventricular sulcus toward the apex. Blue venous companions course in parallel within the same grooves, consistent with tributaries draining to the coronary sinus on the diaphragmatic surface. An inferior view matters because it clarifies the RCA’s relationship to the diaphragmatic right ventricle and the atrioventricular junction, where many clinically relevant branches originate and where variation in dominance becomes visually obvious. Inferior wall myocardial infarction most often reflects occlusion of the RCA (or, in left dominance, the circumflex), and the proximity of RCA perfusion territory to the AV node explains the bradyarrhythmias and varying degrees of AV block that can accompany an inferior STEMI. This perspective also aligns with how operators mentally map the vessel during coronary angiography when interpreting a proximal RCA lesion versus a distal lesion near the crux. Cardiology and anatomy faculty use this type of rendering to teach coronary dominance, the crux, and the typical distribution of the posterior descending artery and posterolateral branches in gross anatomy, pathophysiology, and cath lab correlation sessions. It also suits figure use in interventional cardiology texts, patient education on inferior MI, and surgical planning discussions around right-sided bypass grafting targets.
