Description
Arising from the right coronary artery in the right atrioventricular (coronary) sulcus, the right marginal branch descends along the acute margin of the heart toward the apex on the anterior surface. Its course tracks the lateral border of the right ventricle, running superficial within epicardial fat and accompanied by small cardiac veins that drain toward the coronary sinus system. Superiorly, the ascending aorta and proximal pulmonary trunk sit centrally at the base, while the right atrium lies medial to the right ventricle and the anterior interventricular groove marks the boundary between the ventricles. Red arterial channels and blue venous channels map the coronary circulation across the epicardium. Clinically, the right marginal branch is a practical landmark for right ventricular perfusion, and its caliber and dominance pattern help explain why some patients develop right ventricular infarction with a proximal right coronary artery occlusion. PCI operators often evaluate marginal branches when planning stent placement across the mid-RCA, where side-branch compromise can produce localized ischemia or arrhythmia related to right ventricular free-wall involvement. This view also supports teaching coronary dominance by relating the acute marginal territory to the likely continuation of the RCA toward the crux and posterior descending artery.
