Description
Arising from the left circumflex artery in the left atrioventricular groove, the left marginal branch (obtuse marginal artery) courses inferiorly along the obtuse margin toward the apex on the lateral wall of the left ventricle. From an anterior-leaning left lateral perspective, the left atrial appendage sits superior and anterior to the proximal circumflex, while the left ventricle forms most of the inferior and lateral cardiac silhouette. Small epicardial arterioles radiate distally over the ventricular surface, paired with superficial cardiac veins that track in parallel but drain toward the coronary sinus. This vessel matters because it supplies a substantial share of the lateral left ventricular myocardium, territory that becomes ischemic in left circumflex disease and in selected obtuse marginal lesions. A short segment occlusion here can produce lateral wall hypokinesis and corresponding ST segment changes in lateral leads, and it is a frequent target in percutaneous coronary intervention or bypass grafting (often with a saphenous vein graft) when the dominant circumflex system feeds large obtuse marginal branches.
