Description
Arising from the proximal right coronary artery in the right atrioventricular groove, the right sinuatrial nodal branch courses superiorly and posteriorly toward the junction of the superior vena cava and right atrium, where it supplies the sinoatrial node. From a right lateral (right-side) perspective, the right atrial appendage lies anterior to this vessel’s target region, while the ascending aorta sits medial and superior to the RCA origin. Epicardial veins (including tributaries of the small cardiac vein) run in parallel grooves but remain superficial and blue-coded against the myocardium. Small atrial arterioles fan over the superior right atrium as the SA nodal artery approaches the crista terminalis region. That trajectory matters because the SA nodal artery is frequently at risk during procedures performed around the superior right atrium and cavoatrial junction, including cannulation for cardiopulmonary bypass, right atrial incisions, and some approaches to surgical or catheter-based treatment of atrial arrhythmias. Ischemia of the sinoatrial node from proximal RCA disease or iatrogenic injury can present as sinus bradycardia, sinus pauses, or tachy-brady syndrome. Variant anatomy is common, so seeing the branch in context of the RCA and atrial landmarks helps when correlating operative findings with coronary angiography. Use this asset for coronary circulation teaching in gross anatomy and cardiothoracic surgery courses, for figures in electrophysiology or coronary artery disease chapters that discuss sinus node dysfunction, and for clinical slide decks explaining why RCA lesions can produce rhythm disturbances.
