Right coronary artery anatomy is presented on the epicardial surface, arising from the right aortic sinus at the aortic root and coursing within the right atrioventricular (coronary) sulcus between the right atrium superiorly and the right ventricle inferiorly. Along its proximal segment, the artery gives off a conus (infundibular) branch toward the right ventricular outflow tract and a right marginal branch that descends along the acute margin toward the apex. As the vessel wraps posteriorly toward the crux of the heart, you would expect continuation as the posterior interventricular artery (posterior descending artery) in right-dominant patterns, with atrial and ventricular branches spreading over the right ventricular free wall. Spatial relationships in this view matter because interventional and surgical planning often hinges on where the RCA runs relative to the tricuspid annulus, right ventricular myocardium, and the ostium at the aortic root. Proximal RCA occlusion commonly produces inferior wall myocardial infarction and can involve the AV node artery near the crux, a setup for bradyarrhythmias and varying degrees of AV block. This angle also suits teaching coronary dominance and the practical distinction between the acute margin (right marginal territory) and the posterior interventricular groove (PDA territory).
