Description
Centered in the lower thorax, the heart anchors the pulmonary circulation as the pulmonary trunk exits the right ventricle and bifurcates into the right and left pulmonary arteries beneath the tracheal bifurcation. Along the left hilum, the left pulmonary artery courses horizontally, then gives a branch consistent with the anterior segmental artery to the left upper lobe, passing in close relationship to the segmental bronchus and adjacent pulmonary veins. Red arterial channels and blue venous channels separate the outflow and return pathways, with pulmonary veins converging inferior and anterior to the arterial branches toward the left atrium. Proximal great vessels, including the ascending aorta and aortic arch, provide superior and posterior context behind the pulmonary arterial tree. Isolating the anterior segmental artery matters when you are teaching or planning around segmental anatomy rather than lobar anatomy, because this branch defines perfusion to the anterior segment and marks the level at which vascular control becomes segment-specific. During a left upper lobe segmentectomy, the surgeon often identifies and divides the anterior segmental artery after confirming the segmental bronchus, while preserving neighboring segmental branches to avoid ischemia or unintended parenchymal loss. The same branching pattern also underpins interpretation of CT pulmonary angiography, where a segmental filling defect can localize pulmonary embolus to a defined bronchopulmonary segment and explain wedge-shaped infarction on follow-up imaging.
