Description
Cut through the male kidney in cross section, the renal artery enters medially at the hilum and divides into segmental arteries that pass posterior to the renal pelvis before coursing between the renal pyramids as interlobar arteries. At the corticomedullary junction these vessels arch as arcuate arteries, giving off interlobular (cortical radiate) arteries that ascend toward the capsule and narrow into afferent arterioles supplying individual glomeruli. Renal columns occupy the spaces between pyramids, and their vascular content makes the interlobar trajectory easy to follow. The collecting system sits central and pale by comparison, with minor calyces cupping each papilla and converging into the renal pelvis and proximal ureter. Segmental renal arterial anatomy matters because it is end artery territory. A short, proximal occlusion or an iatrogenic injury during partial nephrectomy can infarct a discrete wedge of parenchyma bounded by adjacent segmental distributions, and this sectional view makes those boundaries intuitive. For endourologic work, the relationship of arteries to the calyces and renal pelvis is a constant planning problem, and the interlobar and arcuate paths explain why bleeding can track along the corticomedullary junction after percutaneous nephrolithotomy. A common teaching point is also visible here: arterioles run with the nephron-bearing cortex, while pyramids concentrate collecting ducts.
