Description
Running superiorly from the left ventricle, the ascending aorta continues into the aortic arch in the superior mediastinum, then gives rise to the brachiocephalic trunk, left common carotid artery, and left subclavian artery as they course superiorly and laterally toward the neck and upper limb. Along the internal surface of the anterior thoracic wall, the internal thoracic (internal mammary) arteries descend parasternal on either side of the sternum, giving anterior intercostal branches that run laterally within the intercostal spaces. Posterior intercostal arteries arise segmentally from the descending thoracic aorta and sweep laterally along the inferior margins of the ribs, paired with veins and accompanied by the intercostal neurovascular bundle. Bony landmarks include the sternum, costal cartilages, ribs, clavicles, scapulae, and the thoracic vertebral column. For teaching thoracic blood supply, an anterior perspective clarifies how the thoracic wall is fed from both anterior and posterior sources and where those systems anastomose across each intercostal space. Surgeons rely on these relationships when harvesting the internal thoracic artery for coronary artery bypass grafting, where parasternal location and proximity to pleura govern dissection planes and bleeding risk. Trauma and iatrogenic injury map to the same anatomy, since posterior intercostal artery laceration can drive a hemothorax, and a misplaced chest tube can intersect the intercostal vessels if introduced near the inferior rib margin.
