Description

Arising from the anterior aspect of each external carotid artery near the level of the greater horn of the hyoid, the superior thyroid arteries descend anteroinferiorly toward the superior pole of the thyroid gland. From the anterior perspective, their course is framed laterally by the sternocleidomastoid and carotid sheath contents, while the laryngeal skeleton and infrahyoid strap muscles sit more medially and superficially over the gland. Early branches are suggested at their origin, including the infrahyoid and superior laryngeal branches, before the main trunks divide along the anterosuperior thyroid surface. Clavicles and the upper thoracic inlet provide inferior bony reference. Surgeons care about this artery because it is encountered early in thyroidectomy and in exposure of the superior pole, where secure ligation must respect the nearby external branch of the superior laryngeal nerve as it approaches the cricothyroid. Injury here can produce loss of high-pitch phonation and vocal fatigue, a complication that is avoidable when the relationship between the artery, the superior pole, and the laryngeal framework is understood. The superior thyroid artery also contributes to collateral flow between external carotid and subclavian systems via anastomoses with the inferior thyroid artery, a practical point in head and neck vascular planning.
