Description
Oriented from the anterior aspect, the cerebrum dominates the field, with the frontal lobes occupying the superior and lateral margins and the interhemispheric (longitudinal) fissure separating the right and left cerebral hemispheres along the midline. Inferiorly, the ventral surface of the frontal lobes leads toward the basal forebrain, where the olfactory bulbs and tracts would lie on the orbital surface, just superior to the nasal cavity. At the lower midline, the brainstem aligns inferiorly toward the foramen magnum, while the cerebellum sits posteroinferior to the cerebrum and is largely obscured from a true anterior look. Midline matters. This anterior perspective is the teaching workhorse for orienting cerebral hemispheric anatomy before moving to sectional planes, because it fixes the student’s sense of left-right symmetry and the clinical habit of describing findings as medial versus lateral and superior versus inferior. Neurologists rely on these surface landmarks when localizing frontal lobe syndromes (abulia, disinhibition, expressive aphasia in the dominant inferior frontal gyrus) and when correlating CT or MRI with deficits following anterior cerebral artery or middle cerebral artery territory infarcts. The depth and continuity of the longitudinal fissure also sets up discussion of falx cerebri relationships and why parasagittal lesions can present with bilateral leg-predominant motor findings.
