Presented as a midsagittal section viewed from a superior perspective, the medial surface of one cerebral hemisphere is opened to expose the deep midline structures of the encephalon. Along the superior margin, the cerebral cortex with its gyri and sulci frames the longitudinal fissure, while more inferiorly the corpus callosum arches in the midline over the diencephalon. The thalamus sits centrally and slightly anterior to the midbrain, with the brainstem continuing inferiorly toward the spinal cord, and the cerebellum positioned posteroinferior to the brainstem; anteriorly, the optic chiasm and optic tract project as a distinct pathway. This superior look at a sagittal cleavage helps you teach and plan around the midline anatomy that dominates neuroimaging and many operative corridors. The relationship of the corpus callosum to the ventricular roof and the diencephalon is a constant reference point when interpreting midline shift, hydrocephalus patterns, or mass effect on CT and MRI. It also maps cleanly onto clinical localization: compression around the optic chiasm by pituitary macroadenoma explains bitemporal hemianopia, while thalamic involvement ties directly to altered arousal and sensory relay deficits. A good midline map matters.
