Description
Cut through the male skull, the cranial cavity frames the encephalon with the cerebral hemispheres occupying the supratentorial compartment and the cerebellum seated posteroinferiorly in the posterior cranial fossa. Prominent gyri and deep sulci contour the cerebral cortex, while the cerebellar hemispheres and vermis present a tighter, foliate surface inferior to the occipital lobes. Along the skull base, the anterior, middle, and posterior cranial fossae step down from anterior to posterior, with the dura mater lining the internal table and draping over ridges such as the petrous temporal bone. Bony foramina punctuate the floor, positioned medial and lateral to the midline landmarks of the sphenoid and occipital bones. Skull base relationships matter because they predict what fails when pathology expands. A posterior fossa mass can compress the cerebellum and brainstem and obstruct cerebrospinal fluid pathways at the level of the fourth ventricle, producing acute hydrocephalus and tonsillar herniation through the foramen magnum. Surgical corridors for suboccipital craniectomy and approaches around the petrous ridge demand a clear mental map of where the posterior cranial fossa sits relative to the tentorium cerebelli and the dural venous sinuses.
