Seen in lateral aspect within a sagittal hemisection of the adult male brain, the basal vein (vein of Rosenthal) courses posteriorly along the medial temporal lobe, hugging the lateral surface of the midbrain in the ambient cistern. Superior to it lie the thalamus and splenium region, while inferior and posterior the cerebellum and dorsal brainstem contour the tentorial notch. Anteriorly, the vein’s tributaries arise around the anterior perforated substance and mesial temporal region, then sweep back toward the vein of Galen at the quadrigeminal cistern. Blue venous channels contrast with red arterial elements around the cerebral peduncles and posterior circulation. This view matters because the basal vein is a key deep venous drainage pathway for the mesial temporal lobe, insula-adjacent deep territories, and midbrain, and its relationship to the tentorial edge and posterior cerebral artery informs both diagnosis and operative corridors. Venous infarction after sacrifice or thrombosis of the basal vein can present with edema or hemorrhage in the thalamus, midbrain, or medial temporal structures, a pattern that can be misread as primary parenchymal disease on CT or MR. For approaches such as subtemporal, occipital transtentorial, or pineal region surgery, this lateral sagittal slice clarifies where venous injury tends to occur. A small vessel, big consequences.
