Running along the inferior free margin of the falx cerebri, the inferior sagittal sinus is shown in lateral profile within a mid-sagittal section of the male brain. Superior to it lies the superior sagittal sinus at the attached convex border of the falx, while posteriorly the inferior sagittal sinus drains toward the straight sinus at the junction with the tentorium cerebelli, continuing to the confluence of sinuses. Medial cerebral landmarks of the sagittal cut are apparent, including the corpus callosum arching superior to the ventricular region, the cerebellum posteroinferiorly, and the brainstem descending to the cervical spinal cord. Cervical vasculature is carried into the same field, with the internal jugular vein and carotid arterial system positioned lateral to the pharyngeal and vertebral column region. Seeing the inferior sagittal sinus in true sagittal relation to the falx clarifies a dural venous channel that is small, variable, and easy to overlook on routine teaching diagrams, yet clinically relevant in parasagittal trauma and falcine masses. A midline interhemispheric approach for anterior falx meningioma, callosal lesion access, or corpus callosotomy risks venous bleeding where the sinus courses along the free edge, and bridging veins from the medial frontal and parietal cortex can complicate exposure. For radiology teaching, this perspective also supports discussion of dural venous sinus thrombosis patterns, where clot extension into the straight sinus can impair deep cerebral venous outflow and produce bilateral thalamic edema on MRI.
