Description
Centered between the orbits in a coronal skull section, the ethmoid bone is presented from an anterior perspective with the cribriform plate spanning transversely at the roof of the nasal cavity and floor of the anterior cranial fossa. Inferior to it, the perpendicular plate descends in the midline to form the superior portion of the nasal septum, positioned medial to the ethmoidal labyrinths. Laterally, the ethmoid contributes to the medial orbital walls via the lamina papyracea, lying immediately medial to the orbital contents and superior to the nasal conchae. Orientation here matters because the ethmoid is where sinonasal and intracranial anatomy meet across a thin bony interface. Even minor trauma can fracture the cribriform plate and tear olfactory filaments, producing anosmia and a CSF rhinorrhea pathway into the nasal cavity. Endoscopic sinus surgery also lives in this neighborhood: violating the lamina papyracea risks orbital fat prolapse and medial rectus injury, while a superior misdirection toward the fovea ethmoidalis can breach the anterior cranial fossa.
