Description
Running along the medial thigh, the gracilis is presented as a narrow, strap-like muscle extending from the body of the pubis near the pubic symphysis inferiorly to its distal insertion on the anteromedial surface of the proximal tibia as part of the pes anserinus. An anterior perspective places the muscle superficial to adductor brevis and adductor magnus, while lying medial to adductor longus proximally and approaching the sartorius tendon near the medial knee. Bony landmarks you would expect in the same field include the pelvis (ilium, ischium, pubis), femur, tibia, and fibula, with the gracilis coursing from proximal medial to distal medial in anatomical position. Clinically, this view helps you teach why medial knee pain can reflect more than an intra-articular problem: pes anserine bursitis and strain at the gracilis insertion often mimic medial meniscal pathology and may coexist with early osteoarthritis. In adductor strains, palpation and resisted hip adduction localize symptoms along the proximal medial thigh, and the gracilis is frequently involved because it crosses both the hip and knee, contributing to hip adduction and knee flexion. Surgeons also use the gracilis as a common autograft or flap, so appreciating its superficial course and distal tendon anatomy is practical for harvest planning and for avoiding saphenous nerve irritation along the medial knee.
