Description
Running from the tibial plateau down to the ankle mortise, the tibia sits medially with the fibula lateral, framing the anterior crural compartment along the interosseous membrane. Across the anterolateral shin you would expect tibialis anterior hugging the lateral surface of the tibia, with extensor hallucis longus and extensor digitorum longus positioned more centrally and laterally as they course toward the dorsum of the foot. Proximally, the contours of the femoral condyles align over the tibial condyles at the knee joint, while distally the malleoli bracket the talus at the tibiotalar articulation. A posterior calf element, such as the gastrocnemius, may be indicated at the margins where it wraps toward the lateral leg. Anterior compartment anatomy matters because it packages the primary dorsiflexors and toe extensors with the deep fibular (peroneal) nerve and anterior tibial vessels, all constrained by unyielding fascia. Compartment syndrome here produces pain with passive toe flexion and can progress to foot drop from deep fibular neuropathy, a pattern clinicians distinguish from common fibular nerve injury at the fibular neck. Tendinopathy of tibialis anterior or extensor hallucis longus, and irritation beneath the superior and inferior extensor retinacula at the ankle, also makes more sense when you can track the tendons to their distal insertions and see their relationship to the tibia and fibula.
