Description

Running in the anterior compartment of the male leg, the extensor digitorum longus (EDL) lies lateral to tibialis anterior and medial to fibularis longus as it descends from the lateral tibial condyle and anterior surface of the fibula. Distally, its muscle belly narrows to a tendon that passes anterior to the ankle joint beneath the superior and inferior extensor retinacula, then divides into slips toward the dorsum of the foot. The tendons course over the metatarsals to the lateral four toes, inserting into the extensor expansions and reaching the middle and distal phalanges. Bony landmarks such as the tibia, fibula, talus, and dorsal midfoot provide clear orientation for the tendon paths. EDL matters because it is a reliable surface and surgical landmark for the deep fibular (peroneal) nerve and anterior tibial artery as they travel between tibialis anterior and EDL in the distal leg. Painful exertional anterior compartment syndrome often presents with dorsiflexion weakness that you can attribute to EDL and its neighbors, while acute deep fibular nerve injury produces foot drop with loss of toe extension that is more specific than a generalized ankle sprain. Clear tendon routing at the ankle also supports teaching of extensor retinacular constraints and why tendon irritation can follow tight bracing or post-traumatic swelling.
