Description

Running in the anterior compartment of the leg, the extensor hallucis longus (EHL) occupies a narrow muscular belly on the anterolateral tibia and anterior surface of the fibula, positioned between tibialis anterior medially and extensor digitorum longus laterally. Distally, its tendon emerges on the dorsum of the ankle beneath the superior and inferior extensor retinacula, then courses toward the hallux to insert on the dorsal base of the distal phalanx. The tibial crest and shaft form the medial bony boundary, while the fibula remains lateral, framing the interosseous interval where the deep fibular (peroneal) nerve and anterior tibial vessels typically travel alongside these extensors. Clinically, this anterior perspective on EHL matters because it anchors the bedside exam for deep fibular nerve function, resisted great toe extension is a quick screen in suspected L4 to L5 radiculopathy or peroneal neuropathy.
