Description
Presented beneath the dorsal skin of the male foot, the extensor digitorum brevis (EDB) occupies the dorsolateral midfoot, its fleshy belly arising from the superolateral calcaneus and the inferior extensor retinaculum just distal to the ankle joint. Medial to it sits the extensor hallucis brevis, while the long extensor tendons of extensor digitorum longus run more centrally toward the lesser toes, crossing superficial to the tarsal bones and metatarsals. Proximally, the tibia and fibula frame the talocrural (ankle) mortise, and distally the EDB tendons course toward the proximal phalanges, lying superficial to the metatarsals and dorsal interossei. Clear landmarks. For clinicians, the EDB region maps directly onto the dorsalis pedis pulse and the course of the deep fibular (peroneal) nerve as they traverse the anterior ankle and dorsal midfoot, so this anterior perspective helps you anticipate where neurovascular structures sit relative to the extensor retinacula and dorsal tendon compartments. Entrapment at the anterior tarsal tunnel commonly produces dorsal foot pain with paresthesia in the first web space, and the EDB can appear denervated or atrophic on exam or EMG when the deep fibular nerve is involved. Surgeons also use the EDB as a local muscle flap to cover dorsal foot defects, and its belly provides an immediate reference when planning incisions for midfoot exposure or hardware placement near the calcaneocuboid and tarsometatarsal joints.
