Description
Running along the superficial anterior forearm, the palmaris longus is shown as a slender fusiform belly arising from the medial epicondyle and continuing distally as a long, cordlike tendon immediately deep to the skin and superficial fascia. The tendon passes superficial to the flexor retinaculum at the wrist and fans into the palmar aponeurosis, lying between flexor carpi radialis laterally and flexor carpi ulnaris medially, with pronator teres crossing obliquely in the proximal forearm. Superficial veins in the subcutaneous tissue are suggested in blue, tracking toward the cephalic and basilic venous pathways along the radial and ulnar margins. Surface anatomy matters here. Palmaris longus is clinically familiar because it is variable, absent unilaterally or bilaterally in a substantial minority, and its prominent tendon becomes an easy landmark when the wrist is flexed against resistance. That relationship is practical in carpal tunnel and distal forearm procedures, where the median nerve lies deep and slightly radial to the palmaris longus tendon while the ulnar neurovascular bundle tracks ulnar to it, and small errors in incision placement can drift into the wrong compartment. Hand surgeons also harvest the palmaris longus tendon for grafting in flexor tendon reconstruction, ulnar collateral ligament reconstruction of the thumb (gamekeeper’s thumb), and various pulley or ptosis procedures because the tendon is long, superficial, and dispensable in most patients. Orthopaedic and anatomy instructors will find this anterior, under-skin rendering useful for teaching forearm surface landmarks, the transition from forearm flexors into palmar fascia, and tendon identification during physical exam. It also fits well in operative atlases and patient education materials that explain tendon graft donor sites and common volar wrist approaches.
