Description
Anteriorly, the sternum is presented as the midline breastbone of the anterior thoracic wall, with the manubrium positioned superiorly, the sternal body extending inferiorly, and the xiphoid process forming the tapered distal end. The jugular (suprasternal) notch sits at the superior border of the manubrium, flanked laterally by the clavicular notches for the sternoclavicular joints. Along each lateral margin, subtle costal notches mark the articulations for the costal cartilages, with the manubriosternal junction forming the palpable sternal angle (Angle of Louis). This anterior perspective matters because the sternum functions as a surface landmark that clinicians use to orient the mediastinum and the ribs, and small errors in identifying the sternal angle can shift rib counting by an entire intercostal space. The manubriosternal joint approximates the level of the 2nd costal cartilage and the T4 to T5 intervertebral disc, a reference level used in describing the aortic arch, tracheal bifurcation, and transverse thoracic plane. Xiphoid morphology also has practical implications; a prominent or bifid xiphoid can be mistaken for an epigastric mass and becomes a hazard during CPR if compressions drift too inferiorly.
