Description

Anteriorly oriented, the T9 vertebra is presented with the vertebral body dominant and the vertebral arch receding posteriorly to form the vertebral foramen. On the superolateral margins of the vertebral body, the superior costal demifacets (for the head of the ninth rib) sit just lateral to the posterior longitudinal ligament attachment zone, while the pedicles course posteriorly toward the laminae. Transverse processes project laterally from the vertebral arch, and the superior and inferior articular processes are indicated by smooth facet surfaces, positioned posterior to the body and medial to the tips of the transverse processes. T9 is a useful mid-thoracic landmark because it participates in costovertebral and zygapophysial joints that shape thoracic cage mechanics and restrict flexion and extension compared with the cervical and lumbar regions. The superior demifacet is a common point of confusion for learners, since thoracic vertebrae typically articulate with two ribs, and the demifacets on adjacent vertebral bodies together receive a single rib head. Fracture morphology and surgical planning also depend on body and pedicle anatomy at this level, where pedicle screw trajectories must respect the medial wall near the spinal canal and the lateral relationship to the rib head and costotransverse region. Small margins matter.
