Description
Running transversely across the anterolateral neck, the transversus colli nerve (transverse cervical nerve, C2 to C3) emerges at the posterior border of the sternocleidomastoid at the classic nerve point and then crosses anteriorly toward the midline. Superficial to the deep cervical fascia, its slender sensory branches course inferior to the mandible and often accompany the external jugular vein as they pass over the sternocleidomastoid. Deep landmarks from the skeleton, including the skull base, cervical vertebrae, clavicles, and upper ribs, anchor the nerve’s relationship to the cervical spine and shoulder girdle. A small nerve with a broad cutaneous territory. An anterior perspective matters because the transverse cervical nerve is routinely encountered in surface anatomy, regional anesthesia, and common surgical exposures along the sternocleidomastoid. It is the primary sensory supply to the skin over the anterior cervical region, and injury during carotid endarterectomy, thyroidectomy extension, cervical lymph node biopsy, or placement of central venous access can produce anterior neck numbness or dysesthesia that patients notice immediately. This view also supports teaching the distinction between the cervical plexus cutaneous branches and nearby motor structures, such as the ansa cervicalis, which lies deeper on or within the carotid sheath region. Faculty can drop this plate into gross anatomy labs, head and neck blocks, or nursing and anesthesia curricula when demonstrating the cervical plexus, Erb’s point, and the relationship of superficial nerves to the sternocleidomastoid and external jugular vein.
