Description
Presented in anterior orientation, the tibia lies medial and larger, while the fibula runs parallel on the lateral side with a narrower shaft and a modestly expanded proximal head. Along the interosseous margin, the fibular border faces medially toward the tibia, defining the lateral limit of the interosseous space that anchors the interosseous membrane. A focal red band marks a fracture line along the fibular diaphysis, distal to the head and neck region. Fibular fractures are often discussed as “minor” because the tibia bears most axial load, but the bone is a key stabilizer for the ankle mortise through the distal tibiofibular syndesmosis and the lateral malleolus. In an anterior view, the relationship of the fracture to the proximal fibular neck matters, because the common fibular (peroneal) nerve winds around this region and is vulnerable in high fibular fractures and in traction injuries associated with varus knee mechanisms. Pattern recognition starts here: an apparently isolated fibular shaft injury can accompany a syndesmotic disruption (Weber C spectrum) or signal a proximal fibular component of a Maisonneuve injury when ankle pain is the presenting complaint. Small bone, big consequences.
