Description
Anteriorly oriented, the aorta descends near the midline from the thoracic cavity into the upper abdomen, where its relationship to the liver’s inflow and outflow becomes the visual anchor. Superior to the hepatic parenchyma, the celiac trunk branches from the abdominal aorta and gives rise to the common hepatic artery, which courses to the right and divides into proper hepatic branches at the porta hepatis. At the same portal region, the portal vein ascends posterior to the hepatic artery and bile duct, while the hepatic veins run superiorly within the liver to drain into the inferior vena cava just inferior to the diaphragm. A short segment of distal esophagus is included, positioned superior and posterior to the left lobe, with periesophageal vessels suggested along its wall. For teaching the great vessels, this anterior perspective is often where learners miss the key point: hepatic arterial blood derives from the aorta through the celiac axis, while portal venous inflow arrives from the splanchnic circulation and the outflow goes directly to the IVC, not back to the portal system. That distinction underpins clinical patterns such as portal hypertension, where portosystemic collateral pathways can enlarge around the distal esophagus (esophageal varices) and at the gastroesophageal junction. It also frames operative planning, since control of the proper hepatic artery and portal vein at the hepatoduodenal ligament is central to the Pringle maneuver during hepatic hemorrhage.
