Viewed from an inferior perspective, the abdominal aorta descends in the retroperitoneum anterior to the vertebral column and ends at its bifurcation into the right and left common iliac arteries at about the L4 level. The aortic trunk sits medial to the psoas major muscles, with the common iliac arteries diverging inferolaterally toward the pelvic brim. Depending on the crop, the inferior vena cava may be suggested to the patient’s right, while the distal aorta remains the dominant midline landmark. A clean bifurcation. Orientation from below matters because the aortic bifurcation is a frequent landing zone for pathology and intervention. Atherosclerotic occlusive disease at the aortoiliac segment (Leriche syndrome) and infrarenal abdominal aortic aneurysm planning both depend on understanding the relationship between the distal aorta, the common iliac origins, and the adjacent vertebral level.
