Description
Framed in anterior anatomical position, the esophagus descends in the midline to join the stomach in the left upper quadrant, with the liver occupying the right hypochondrium and extending across the epigastrium to overlie the gastric fundus and body. Inferior to the stomach, the coiled small intestine fills the central abdomen, while the large intestine forms a peripheral frame with ascending colon on the right, transverse colon crossing superiorly, and descending colon on the left. Tucked on the visceral (inferior) surface of the right lobe, the gallbladder sits just inferior to the liver margin, aligned to drain bile into the proximal small bowel. Spatial relationships are clear. This anterior arrangement is the one clinicians map onto surface anatomy: liver dullness to percussion under the right costal margin, gastric bubble in the left upper quadrant, and colonic gas outlining the periphery on radiographs. The overlap of the liver over the stomach matters in blunt abdominal trauma and in operative planning, since subcostal approaches and laparoscopic port placement must account for hepatic edge position and the gallbladder fossa. It also supports teaching of referred pain patterns, such as biliary colic presenting in the right upper quadrant with radiation to the right scapula.
