Abdominal adiposity increases the depth of the anterior abdominal wall, so the viscera sit deeper relative to the surface landmarks while maintaining their normal quadrant relationships. Obesity changes how clinicians think about palpation, imaging windows, and operative access to the alimentary tract. A deeper, more adipose abdominal wall can blunt physical exam findings in acute abdomen and makes sonographic evaluation of hepatobiliary disease more limited, shifting reliance toward CT in many emergency pathways.
