Description
Centered in the anterior torso, the esophagus descends through the diaphragm to join the stomach in the left upper quadrant, while the liver occupies the right upper quadrant and extends medially over the gastric region. Inferior to the stomach, coils of small intestine (jejunum and ileum) fill the central abdomen, framed peripherally by the large intestine with the ascending colon on the right, transverse colon coursing superiorly, and descending colon on the left toward the sigmoid colon. Abdominal adiposity increases the depth of the anterior abdominal wall, so the viscera sit deeper relative to the surface landmarks while maintaining their normal quadrant relationships. Obesity changes how clinicians think about palpation, imaging windows, and operative access to the alimentary tract. A deeper, more adipose abdominal wall can blunt physical exam findings in acute abdomen and makes sonographic evaluation of hepatobiliary disease more limited, shifting reliance toward CT in many emergency pathways. For general and bariatric surgeons, this anterior arrangement also highlights why laparoscopic port placement and retraction strategies differ in patients with central obesity, and why the left lobe of the liver can become a prominent obstacle during exposure of the stomach and proximal duodenum.
