Description
Across the anterior abdomen of an obese adult male, the esophagus descends through the diaphragmatic hiatus to the stomach, whose fundus and greater curvature sweep inferolaterally toward the left upper quadrant in close apposition to the spleen. Superior and to the right, the liver occupies the right hypochondrium with the gallbladder tucked on its inferior surface, while the pancreas runs transversely posterior to the stomach toward the splenic hilum. Coiled jejunum and ileum fill the central abdomen, framed peripherally by the colon, with ascending colon on the right, transverse colon crossing superiorly, and descending colon on the left. An anterior perspective is a practical way to teach surface anatomy and organ adjacency in obesity, where abundant subcutaneous and visceral fat can alter the apparent position of the stomach and transverse colon and blunt palpable landmarks on exam. The stomach’s greater curvature and its relationship to the spleen matter during sleeve gastrectomy and other bariatric procedures, when mobilization of the greater curvature risks injury to short gastric vessels and can contribute to postoperative bleeding near the splenic capsule.
