Description
Running from the right side of the superior mesenteric artery toward the ileocecal region, the ileocolic artery descends obliquely and then divides into ileal, colic, cecal, and appendicular branches distributed along the terminal ileum and proximal ascending colon. An anterior abdominal view places the vessel within the mesentery, anterior to the posterior abdominal wall, with the cecum and ileocecal junction inferior and right-sided relative to the transverse colon. Companion ileocolic veins course alongside the arterial branches, converging toward the superior mesenteric vein, while the marginal artery of the colon can be inferred along the medial border of the ascending colon near the hepatic flexure. Portions of the stomach and inferior margin of the liver sit superiorly, and the pelvic brim provides inferior bony reference. For colorectal surgeons, this is the pedicle. High ligation of the ileocolic vessels during right hemicolectomy or ileocecal resection hinges on identifying the ileocolic artery’s origin from the superior mesenteric artery and separating it cleanly from adjacent mesenteric fat, lymphatics, and the superior mesenteric vein. Variants matter, including a prominent appendicular artery or shared trunks with the right colic artery, because they influence perfusion of the ileal stump and the risk of ischemia at an ileocolic anastomosis. Use this artwork in gross anatomy and GI surgery teaching to orient students to mesenteric arterial branching, or in operative atlases discussing medial-to-lateral mobilization of the right colon and vascular control at the ileocolic pedicle.
