Description
Prominent in the lower abdomen, the abdominal aorta descends anterior to the lumbar vertebral bodies and divides near the level of L4 into the right and left common iliac arteries. Each common iliac artery courses inferolaterally along the medial border of the psoas major toward the pelvic brim, then bifurcates into the internal iliac artery medially and the external iliac artery laterally, just anterior to the sacroiliac region. Superior bony landmarks include the costal margins and lower ribs, with the upper pelvis framing the vascular bifurcation region. The inferior vena cava runs to the right of the aorta, providing immediate left right orientation. Clinically, the aortoiliac bifurcation is a common site for atherosclerotic occlusive disease and thromboembolism, and this anterior orientation helps you relate limb ischemia patterns to proximal inflow lesions in the common or external iliac artery. Endovascular planning also hinges on these relationships, since aortic bifurcation stents, iliac kissing stents, and access from the common femoral artery all depend on the external iliac trajectory under the inguinal ligament while preserving internal iliac flow to reduce buttock claudication and pelvic ischemic complications. A key teaching point. Internal iliac compromise also matters during pelvic surgery and embolization for trauma or postpartum hemorrhage. Use this illustration in gross anatomy and pelvic anatomy teaching to anchor the transition from abdominal to pelvic vasculature, and in vascular surgery or interventional radiology materials when explaining aortoiliac disease, stent placement, or catheter navigation to the internal iliac branches.
