Description
Arising from the abdominal aorta at its bifurcation around the L4 vertebral level, the right and left common iliac arteries run inferolaterally along the medial border of the psoas major toward the pelvic brim. Each vessel is shown as a short, broad-caliber trunk before dividing into the internal iliac artery, which turns medially into the pelvis, and the external iliac artery, which continues distally along the pelvic inlet toward the femoral canal. Blue companion veins flank the arterial tree, with the common iliac veins positioned posteromedial to their corresponding arteries and converging superiorly to form the inferior vena cava. Posterior skeletal landmarks, including lumbar vertebrae and the inferior rib cage, anchor the vascular relationships. Clear depiction of the common iliac segment matters because so much operative and endovascular planning hinges on millimeters at the aortic bifurcation. Aneurysmal extension into the common iliac arteries influences stent-graft landing zones during EVAR, and preservation of internal iliac flow is a constant concern when treating iliac occlusive disease or aneurysm, given the risk of buttock claudication and pelvic ischemia after embolization or coverage. Short and wide by design, the common iliac arteries also sit close to the lumbosacral trunk and the iliac veins, a practical reminder of venous injury risk during anterior approaches to the lumbosacral spine and pelvic brim exposures. Tight anatomy. Use this illustration in vascular surgery and interventional radiology teaching to orient learners to the aortoiliac axis before discussing aortoiliac occlusive disease, iliac aneurysm repair, or pelvic collateral pathways.
