Description
Superficial to the anterior forearm fascia, the palmaris longus belly is positioned in the superficial flexor compartment, arising from the medial epicondyle and running distally between flexor carpi radialis laterally and flexor carpi ulnaris medially. Its slender tendon becomes prominent in the distal third of the forearm, crosses anterior to the flexor retinaculum at the wrist, and blends with the central fibers of the palmar aponeurosis in the proximal palm. Deep to this line of pull, the radius lies lateral to the ulna, with the carpal bones forming a stacked arch proximal to the metacarpals and phalanges. For surface anatomy, this anterior, subcutaneous rendering matters because palmaris longus is both variable and clinically exploited: it is absent unilaterally or bilaterally in a sizable minority of patients, and its presence changes what you can palpate at the volar wrist. Surgeons commonly harvest the tendon for grafting (for example, thumb ulnar collateral ligament reconstruction and ptosis repair), so clear depiction of its course relative to the flexor retinaculum and palmar fascia helps communicate incision planning and expected donor-site anatomy.
