Fibers arise from the hook of the hamate and adjacent flexor retinaculum, then course anterior to the fifth metacarpophalangeal joint alongside the abductor digiti minimi, with the opponens digiti minimi deeper against the metacarpal shaft. The palmar aponeurosis and hypothenar fascia frame the muscle belly, while the flexor tendons to the digits traverse more centrally in the palm under fibrous sheaths. Functionally, this muscle produces flexion at the fifth metacarpophalangeal joint and contributes to ulnar-sided grip, a point that becomes obvious when comparing power grip versus precision pinch mechanics.
