Description
Viewed from an inferior aspect at the cranial base, the superior petrosal sinus courses posterolaterally along the superior border of the petrous part of the temporal bone, bridging the cavernous sinus anteriorly to the transverse sinus posteriorly at the petro-occipital region. Adjacent dural venous channels at this level typically include the cavernous sinus on either side of the sella region and the transverse sinus along the posterolateral margin, with the tentorium cerebelli attaching along the petrous ridge that the sinus follows. Yellow cranial nerves emerge from the brainstem and traverse the prepontine and cerebellopontine angle cisterns, lying medial and inferior to the petrous ridge as the venous sinus tracks above them in the tentorial attachment. Tight anatomy. For skull base teaching, this is the money relationship: the superior petrosal sinus sits in the petrous attachment of the tentorium, directly relevant to approaches that mobilize or incise the tentorium near the petrous apex. Injury here can complicate anterior petrosectomy or combined petrosal approaches with brisk venous bleeding, and its junctional anatomy explains how cavernous sinus thrombosis can propagate posteriorly into the transverse-sigmoid system. The inferior perspective also clarifies why petrous ridge meningiomas and tentorial lesions may displace this sinus and alter venous drainage patterns.
