Description
Positioned on the inferior surface of the liver, the gallbladder is shown continuous with the cystic (cholecystic) duct at its neck, where circumferential smooth muscle thickening forms the functional gallbladder sphincter, commonly described as the spiral folds or valve of Heister. Proximally, the cystic duct courses toward the common hepatic duct to form the common bile duct, placing the cholecystic outflow mechanism in direct continuity with the extrahepatic biliary tree. Expect the nearby hepatic parenchyma superiorly and the gallbladder fundus projecting anteriorly and inferiorly relative to the neck. Small structure, big consequences. Understanding this sphincteric mechanism matters when you are teaching regulated bile delivery and gallbladder filling, because resistance at the cystic duct contributes to gallbladder distension during fasting and influences pressure dynamics when the common bile duct is transiently obstructed. It also provides an anatomical anchor for clinical discussion of biliary colic and cholecystitis, where cystic duct obstruction by a gallstone at or near the neck can trap bile and mucus, produce hydrops, and complicate identification of the cystic duct during dissection of Calot’s triangle in laparoscopic cholecystectomy. Clear depiction of the cholecystic duct’s internal folds helps explain why cannulation can be difficult during ERCP or intraoperative cholangiography.
