Description
Positioned on the visceral (inferior) surface of the liver, the body of the gallbladder appears as the central, expanded segment between the fundus anteriorly and the infundibulum (Hartmann pouch) and neck proximally toward the porta hepatis. From a posterior orientation, the hepatic surface of the gallbladder lies apposed to the gallbladder fossa, with the peritoneal reflection absent where it contacts hepatic parenchyma and present along the free, inferior margin. Superiorly the body relates to hepatic segments IVB and V, while inferiorly it faces the transverse colon and proximal duodenum across the peritoneal cavity. Concentric smooth muscle of the gallbladder wall is implied beneath the serosa. Short axis anatomy matters here. Posterior orientation is the angle that best explains why gallbladder disease so often becomes a hepatobiliary problem instead of an isolated cystic structure. When cholecystitis progresses, inflammation and adhesions commonly involve the hepatic bed and can tether the gallbladder body to the liver, complicating dissection during laparoscopic cholecystectomy. This view also supports teaching the path of bile as it exits the neck into the cystic duct, where an impacted stone at Hartmann pouch can create Mirizzi syndrome by compressing the common hepatic duct.
