Description
Oriented to the upper thorax, the right and left superior lobes (lobus superior pulmonis) are presented as tapering cones rising to the apices, with their broad bases resting on the diaphragmatic surface. On the right, the superior lobe lies anterior and superior to the oblique fissure and adjacent to the horizontal fissure that separates it from the middle lobe, while the left superior lobe is bounded inferiorly by the oblique fissure and continues anteriorly into the lingula. Along the mediastinal surfaces, the lobes contour around the hilum and root of the lung, with the apices projecting superior to the first rib into the cervical pleura. Clear left-right asymmetry. This anatomy matters because many high-yield clinical and imaging findings localize to the cranial (upper) lobes and their fissural boundaries. Primary (reactivation) tuberculosis and apical fibrobullous disease commonly favor the superior lobes, so teaching their apical extent relative to the thoracic inlet helps explain why cavitation or pneumothorax may present with subtle early signs. The fissures also guide segmental localization on chest radiographs and CT, and they define the operative planes for upper lobectomy and video-assisted thoracoscopic surgery, where incomplete fissures can increase air leak risk.
