Description
Seen from a superior (cranial) directional viewpoint, the apical portions of both lungs occupy the most superior aspect of the thorax, flanking the superior mediastinum on either side. Color-coding separates the superior lobes from adjacent parenchyma, with the right lung expected to show the horizontal fissure dividing superior and middle lobes, and the left lung aligning anteriorly with the cardiac notch region and lingula inferior to the apex. Superiorly, the lung apices project above the level of the first rib into the root of the neck, while the costal surfaces slope inferolaterally toward the bases. A top-down perspective matters because apical lung anatomy is where thoracic and cervical landmarks meet, and where clinical problems often present with nonthoracic symptoms. A Pancoast (superior sulcus) tumor at the lung apex can invade the brachial plexus and cervical sympathetic chain, producing shoulder and arm pain with Horner syndrome, so a clean depiction of the cranial extent of the lobes helps teach why these findings localize. This orientation also supports correlation with axial CT at the thoracic inlet, where small differences in fissure position and lobe boundaries alter segmental localization. Use this artwork in gross anatomy and respiratory system teaching modules to anchor lobe identification before moving to cross-sectional imaging, and in radiology or oncology publications discussing apical masses, thoracic inlet anatomy, and lobar localization on CT.
