Description

Superior (upper, cranial) pulmonary lobes are positioned at the apices of the lungs, extending from the thoracic inlet down to the oblique fissures. On the right, the superior lobe lies superior to both the horizontal fissure and the major (oblique) fissure, while on the left it sits superior to the oblique fissure and anterior to the hilum. Medially, each lobe approaches the mediastinum and root of the lung, and laterally it conforms to the inner surface of the rib cage. Fissures define the borders. Upper-lobe anatomy matters because ventilation, perfusion, and disease distribution are not uniform across the lungs. Reactivation tuberculosis and many cavitating infections favor the apical and posterior segments of the superior lobes, a pattern that influences chest radiograph interpretation and bronchoscopy planning. In thoracic surgery, understanding the relationship of the right superior lobe to the horizontal fissure and the left superior lobe to the lingula helps anticipate fissure completeness, air-leak risk, and the operative planes during lobectomy or segmentectomy. Segmental orientation is the difference between a clean dissection and a prolonged case.
