Description
Anterior orientation centers on the hepatopancreatic ampulla (ampulla of Vater) at the posteromedial wall of the second part of the duodenum, where the ductus choledochus and main pancreatic duct converge before opening at the major duodenal papilla. The common bile duct descends from superior and rightward to meet the pancreatic duct approaching from the left within the head of the pancreas, creating a short shared terminal channel. Surrounding duodenal mucosa and the sphincteric complex at the ampulla are implied landmarks even when simplified. Small structure, big consequences. Clinically, this junction explains why a single obstructing gallstone at the ampullary termination can produce both obstructive jaundice and acute pancreatitis, while a periampullary carcinoma may present with painless jaundice and early biliary dilatation. Endoscopic retrograde cholangiopancreatography targets the major papilla from the duodenal lumen, and a clear anterior observation helps trainees understand how cannulation angles relate to the ductus choledochus versus the pancreatic duct.
