Description
Centered in the upper right abdomen, the supraduodenal part of the ductus choledochus (common bile duct) runs inferiorly within the free edge of the lesser omentum, positioned anterior to the portal vein and typically lateral to the proper hepatic artery. Superiorly it continues from the junction of the ductus hepaticus communis and ductus cysticus, while inferiorly it descends toward the first (superior) part of the duodenum. Nearby anterior relationships commonly include the visceral surface of the liver and the peritoneal reflections of the hepatoduodenal ligament. A short, high-yield segment. Anterior visualization of this segment matters because it is the portion most directly encountered when you open the hepatoduodenal ligament during a Pringle maneuver, where the portal triad is compressed to control hepatic inflow bleeding. The same relationships explain both the risk profile of common bile duct injury during cholecystectomy (misidentification near the cystic duct junction) and the pattern of obstruction in choledocholithiasis, where a stone lodged distally causes upstream biliary dilatation and jaundice. For operative planning, the anterior view clarifies which structure lies in front and which is protected posteriorly. Useful for hepatobiliary anatomy teaching in gross anatomy and surgical clerkships, and for illustrating the portal triad in operative atlases, ERCP-related education, or patient-facing explanations of extrahepatic biliary obstruction.
