Description
Anterior anatomy centers on the hepatopancreatic ampulla (ampulla of Vater) as the ductus choledochus and ductus pancreaticus converge and enter the second part of the duodenum at the major duodenal papilla. Surrounding this junction, the sphincter of the ampulla (sphincter of Oddi) encircles the terminal segments of both ducts, with distinct choledochal and pancreatic components lying proximal to the shared channel. Superior and posterior to the papilla sits the head of the pancreas, while the duodenal wall forms the immediate muscular envelope around the intramural ducts. Small but decisive. Functionally, this arrangement explains how biliary and pancreatic outflow can be obstructed without an upstream stone. Basal sphincter tone and dyskinesia underlie sphincter of Oddi dysfunction, a post cholecystectomy pain syndrome that can present with episodic biliary colic, transient cholestatic enzyme elevation, or pancreatitis when the pancreatic sphincter component is involved.
