Description
Anteriorly oriented anatomy centers on the gallbladder neck and the proximal ductus cysticus, where the mucosa forms the spiral folds (valvulae spirales, valves of Heister) within the lumen. Circumferential smooth muscle at the neck, commonly described as the sphincter of Lütkens, encircles this transition zone, lying inferior to the liver’s visceral surface and medial to the fundus as the gallbladder tapers into the cystic duct. A short segment of the cystic duct extends distally from the neck toward the porta hepatis, its spiral valve ridges running obliquely along the duct wall. Small peritoneal reflections around the gallbladder fossa help anchor orientation against the anterior abdominal wall. Clinically, this junction is where functional outflow resistance and mechanical obstruction overlap. Cystic duct stones frequently lodge at the neck or in the proximal cystic duct, producing biliary colic and acute cholecystitis, and the presence of prominent valvulae spirales can make cystic duct cannulation or contrast passage during intraoperative cholangiography frustrating.
