Anterior Surface Anatomy of the Anterior Interventricular Sulcus

Description

Running along the anterior (sternocostal) surface of the heart, the anterior interventricular sulcus marks the external boundary between the right ventricle on the viewer’s right and the left ventricle on the viewer’s left as it descends toward the apex. Superiorly, the groove begins near the coronary (atrioventricular) sulcus at the base of the ventricles, then continues inferiorly in a longitudinal course. Flanking myocardium from both ventricles forms the raised muscular contours around this interventricular furrow. Clean surface anatomy. Clinically, this sulcus is the surface landmark for the anterior interventricular artery (left anterior descending, LAD) and the great cardiac vein, structures that course in the groove on their way toward the coronary sinus. Correlating the external groove with the underlying interventricular septum helps when teaching why LAD occlusion produces an anterior wall and septal myocardial infarction, often with characteristic precordial ECG changes and risk to the anterior papillary muscle territory. Surgeons and interventionalists also use this topography to orient on the beating heart during coronary artery bypass grafting and when describing lesions on coronary angiography.
