Description
Anteriorly oriented on the cardiac mass, the anterior interventricular sulcus (sulcus interventricularis anterior) runs longitudinally from the coronary (atrioventricular) sulcus toward the apex cordis, marking the surface plane between the right and left ventricles. The anterior wall of the right ventricle lies to the viewer’s right and tends to dominate the sternocostal surface, while the left ventricle forms the left cardiac border and contributes the thicker myocardium approaching the apex. Superiorly, the groove begins just inferior to the arterial roots, with the base of the heart positioned cranially and the apex directed inferiorly and to the left. Removing the coronary vessels from this field is deliberate. It forces attention onto the sulcus as a topographic landmark rather than as a carrier for the left anterior descending artery (anterior interventricular branch of the left coronary artery) and great cardiac vein, which normally occupy this furrow and can obscure its depth and course. In the operating room and in cardiac imaging correlation, that course matters: the sulcus approximates the interventricular septum and guides orientation during anterior approaches, and it frames the myocardial territory at risk in anterior wall ischemia when the LAD is occluded. A clean groove reads like a map. Use this illustration when you need to teach surface anatomy of the ventricles in gross anatomy, cardiothoracic surgery orientation sessions, or exam-prep materials that contrast external landmarks with internal septal anatomy. It also suits publishers building diagrams for ECG and myocardial infarction territory discussions, where the LAD is referenced but the underlying sulcus must be understood first.
