Description
Running vertically on the anterior surface of the right atrium, the sulcus terminalis forms a shallow external groove that corresponds internally to the crista terminalis. Medial to the groove lies the smooth-walled sinus venarum receiving the superior and inferior venae cavae, while lateral to it the atrial wall becomes more trabeculated with pectinate muscles extending toward the right atrial appendage. Superiorly the sulcus approaches the cavoatrial junction near the right side of the ascending aorta, and inferiorly it trends toward the IVC orifice and the atrioventricular (coronary) sulcus. Clinically, this landmark matters because the sulcus terminalis tracks the line of embryologic fusion between the primitive atrium and the sinus venosus, a boundary that helps explain the abrupt transition from smooth posterior atrial wall to the muscular appendage. Electrophysiologists reference this region during mapping and ablation, since the crista terminalis can act as a conduction barrier and a substrate for atrial tachycardia circuits, and the nearby sinoatrial node sits at the superior end close to the SVC. Small groove, big implications.
