Description
Running vertically on the right cardiac border, the sulcus terminalis forms a shallow external groove on the right atrium, extending from near the superior vena cava region superiorly toward the inferior vena cava region inferiorly. Anterior to the groove lies the rough-walled atrium proper, corresponding internally to the pectinate muscle-bearing appendage, while posterior to it sits the smoother sinus venarum. The right atrioventricular (coronary) sulcus defines the inferior margin of the atrium where it meets the right ventricle. No vessels interrupt the surface anatomy. Clinically, the sulcus terminalis matters because it marks the external correlate of the crista terminalis, the internal ridge separating the embryologic sinus venosus from the primitive atrium. This is a common anatomic reference in right atrial electrophysiology: the sinoatrial node lies subepicardially near the superior end of the sulcus at the cavoatrial junction, and catheter mapping or ablation around the crista terminalis must account for this relationship. Orientation to this groove also helps trainees reconcile why the posterior right atrial wall is smooth on echo and intraoperative views while the anterolateral wall and appendage show prominent pectinate musculature.
